An excellent opportunity is available for experienced Nurse Practitioners to provide home health risk assessments on a per diem basis. This flexible role involves conducting comprehensive evaluations for Medicare Advantage plan members in Wise County, including Big Stone Gap, Wise, and Coeburn. Practitioners will have the ability to design their own work schedules, choosing shifts any day of the week between Monday and Sunday, with options for minimum weekly or bi-weekly commitments.
Schedule:
- Flexible shifts available Monday through Sunday
- Set your own working hours
- Minimum required weekly or bi-weekly hours
Responsibilities:
- Perform annual or post-discharge in-home health risk assessments, averaging 45 minutes per visit
- Review and evaluate prior and current medical history, conditions, and medication regimens
- Educate patients on health conditions and prescribed medications
- Identify care gaps, addressing and recommending solutions or follow-up as needed
- Focus exclusively on assessment: no treatment plans or medication prescribing required
Qualifications:
- Active Nurse Practitioner license issued by the state of Virginia
- Board Certification (AANP or ANCC)
- Current Basic Life Support (CPR) certification
- MSN degree from an accredited institution
- Minimum of six months Nurse Practitioner experience preferred
Nurse Practitioners in this role will be compensated at $80-$90 per evaluation, with additional mileage and administrative time stipends.
