Step into an impactful, flexible per diem opportunity as a Nurse Practitioner conducting home health risk assessments across Androscoggin County. In this role, you will visit Medicare Advantage plan members at their residences, providing valuable annual health assessments or post-discharge check-ups in locations such as Auburn, Lisbon, Poland, Lewiston, and surrounding areas.

Schedule:

• Flexible shifts available any day from Monday to Sunday
• Ability to create your own schedule
• Minimum weekly or bi-weekly hourly commitment required

Key Responsibilities:

• Perform comprehensive home health risk assessments, typically lasting about 45 minutes per visit
• Review and document prior and current medical history, ongoing medical conditions, and detailed medication records
• Offer clear education on medical conditions and medication management for plan members
• Identify gaps in care and suggest actionable recommendations to improve health outcomes
• Focus solely on assessmentno prescribing or development of treatment plans involved

Qualifications:

• Active Nurse Practitioner license in the state of ME
• Current BLS (CPR) certification
• Board Certification through AANP or ANCC required
• Minimum of 6 months experience working as a Nurse Practitioner preferred
• MSN from an accredited school of nursing
