Deliver essential healthcare services to the community by conducting home health risk assessments for Medicare Advantage plan members across Pitt County, including Greenville, Winterville, and Ayden. This opportunity offers a per diem, highly flexible schedulework shifts that suit your availability, with the ability to create your own hours between Monday and Sunday. Minimum weekly or bi-weekly hourly commitments ensure you maintain professional balance while making a meaningful difference.
Key Responsibilities:
- Conduct comprehensive annual home health risk assessments as part of a Medicare Advantage health plan
- Review and evaluate patients prior/current medical and medication histories
- Educate patients regarding medical conditions, prescribed medications, and overall health
- Identify gaps in healthcare or necessary follow-up, and provide actionable recommendations
- Complete documentation accurately and timely; no requirement to develop treatment plans or prescribe medications
Required Qualifications:
- Active Nurse Practitioner license in North Carolina
- Certification in Basic Life Support (CPR/BLS)
- Board Certification through AANP or ANCC
- Master of Science in Nursing (MSN) degree from an accredited school
- At least 6 months of recent NP experience preferred
