Job Description
Deliver advanced clinical care and assessments in a dynamic setting, focusing on home health risk evaluations for Medicare Advantage plan members. As a Nurse Practitioner specializing in Home Health, you will play a crucial role in ensuring high-quality care by visiting patients in their homes throughout Beaufort County, including areas such as Bath, Pantego, and Pinetown. This per diem opportunity offers exceptional flexibility and autonomy in scheduling.
Schedule:
- Flexible shifts available Monday through Sunday
- Self-created schedules for optimal work-life balance
- Minimum weekly or bi-weekly commitment of hours
Core Duties:
- Conduct comprehensive home health risk assessments, each lasting approximately 45 minutes
- Evaluate detailed medical history, ongoing medical conditions, and medication usage
- Educate patients and families about medical conditions, prescribed medications, and best self-care practices
- Identify care gaps and proactively recommend actions for resolution
- Visits vary, including annual health assessments and post-discharge evaluations
- No requirements for creating treatment plans or prescribing medications
Ideal Qualifications:
- Active Nurse Practitioner License in North Carolina
- Board Certification (AANP or ANCC)
- Current CPR certification (BLS)
- Minimum of six months recent experience as a Nurse Practitioner is highly preferred
- Master of Science in Nursing from an accredited institution
In this role, you will be compensated $80$90 per evaluation, with mileage stipend and administrative time included, recognizing the importance of your expertise and commitment to patient care.
