Job Description
This per diem role offers Nurse Practitioners the opportunity to deliver meaningful, patient-centered care outside of a traditional clinical environment by conducting home health risk assessments within Pitt County. Providers in this position visit Medicare Advantage plan members in their own homes, supporting them through either annual health evaluations or post-discharge visits. Each assessment typically takes about 45 minutes and focuses on both health history review and proactive education.
Schedule/Hours
- Flexible shifts are available Monday through Sunday
- You have the freedom to create your own schedule
- Minimum commitment is based on weekly or bi-weekly hours
Responsibilities
- Complete in-home annual health risk assessments for Medicare Advantage participants
- Review and evaluate patients current and prior medical histories, as well as medication usage
- Offer education and guidance on health conditions and medications directly to each member
- Identify gaps in care and suggest actionable recommendations to close those gaps
- No prescribing or development of treatment plans is required; this role is focused on assessment and education
Qualifications
- Active Nurse Practitioner license in North Carolina
- Current CPR (BLS) certification
- Board-certified through AANP or ANCC
- At least six months of recent NP experience is preferred
- MSN from an accredited nursing program
This unique per diem opportunity includes compensation of $80$90 per assessment, in addition to a mileage stipend and administrative time.
