Step into a dynamic home health environment where your expertise as a Nurse Practitioner can make a significant impact on patient well-being. This per diem opportunity offers autonomy and flexibilityallowing you to create your own schedule and work throughout Lake County, FL, including Clermont, Leesburg, and Eustis. As a key member of the home health team, you will perform comprehensive risk assessments for Medicare Advantage plan members, helping to identify health concerns and improve outcomes.
Schedule:
- Flexible shifts available Monday through Sunday
- Set your own working hours
- Weekly or bi-weekly minimum hour commitment required
Key Responsibilities:
- Conduct in-person annual home health risk assessments (approximately 45 minutes per visit)
- Review and document prior and current medical history, conditions, and medication use
- Educate patients on chronic health conditions and medication management
- Identify gaps in care and offer actionable recommendations to address them
- Visits may include annual assessments or post-discharge evaluations
- No responsibility for creating treatment plans or prescribing medications
Qualifications:
- Active Nurse Practitioner license in Florida
- Current BLS/CPR certification
- Board certification through AANP or ANCC required
- MSN degree from an accredited school of nursing
- At least 6 months current NP experience preferred
Your efforts will be rewarded with compensation of $80$90 per evaluation, alongside mileage reimbursement and payment for administrative time.
