Step into a dynamic home health environment where your expertise as a Nurse Practitioner can make a significant impact on patient well-being. This per diem opportunity offers autonomy and flexibilityallowing you to create your own schedule and work throughout Lake County, FL, including Clermont, Leesburg, and Eustis. As a key member of the home health team, you will perform comprehensive risk assessments for Medicare Advantage plan members, helping to identify health concerns and improve outcomes.

Schedule:

• Flexible shifts available Monday through Sunday
• Set your own working hours
• Weekly or bi-weekly minimum hour commitment required

Key Responsibilities:

• Conduct in-person annual home health risk assessments (approximately 45 minutes per visit)
• Review and document prior and current medical history, conditions, and medication use
• Educate patients on chronic health conditions and medication management
• Identify gaps in care and offer actionable recommendations to address them
• Visits may include annual assessments or post-discharge evaluations
• No responsibility for creating treatment plans or prescribing medications

Qualifications:

• Active Nurse Practitioner license in Florida
• Current BLS/CPR certification
• Board certification through AANP or ANCC required
• MSN degree from an accredited school of nursing
• At least 6 months current NP experience preferred

Your efforts will be rewarded with compensation of $80$90 per evaluation, alongside mileage reimbursement and payment for administrative time.
