Schedule:

Key Responsibilities:

  • Conduct comprehensive annual home health risk assessments for Medicare Advantage health plan members
  • Evaluate patients current and past medical histories, chronic conditions, and medication lists
  • Educate individuals on their medical conditions and prescribed medications
  • Identify and address gaps in care, providing clear recommendations and follow-up instructions
  • Visits do not require prescribing medication or creating new treatment plans

Qualifications:

  • Valid and unrestricted Nurse Practitioner license in the state of Florida
  • Master of Science in Nursing from an accredited institution
  • Board Certification as a Nurse Practitioner (AANP or ANCC)
  • Current Basic Life Support (CPR/BLS) certification
  • At least 6 months of recent Nurse Practitioner experience is preferred

In this per diem position, youll enjoy compensation of $80-$90 per completed evaluation, along with a mileage stipend and payment for administrative time.
