Step into a dynamic per diem opportunity as a Nurse Practitioner specializing in home health risk assessments for Medicare Advantage plan members throughout Forsyth County, NC. Youll serve clients in areas such as Kernersville, Clemmons, and Lewisville, completing assessments that help improve patient outcomes without the need to create treatment plans or prescribe medications. This role offers exceptional flexibilitycraft your own schedule with shifts available any day of the week and a minimum weekly or bi-weekly hourly commitment to suit your lifestyle.
Schedule:

Key Responsibilities:
- Conduct comprehensive annual risk assessments in home settings
- Evaluate patients prior and current medical history, ongoing conditions, and medication usage
- Deliver education on health conditions and medication management
- Spot gaps in care and advise on solutions
- Focus exclusively on assessmentsno treatment planning or medication prescribing required
Qualifications:
- Active Nurse Practitioner License in North Carolina
- Board certification via AANP or ANCC
- CPR certification (BLS) required
- MSN from an accredited school of nursing
- At least 6 months of current nurse practitioner experience preferred
