Advance your career as a Nurse Practitioner by delivering high-quality care through home health risk assessments for Medicare Advantage plan members. This per diem opportunity offers flexible scheduling and a chance to make a difference in the lives of individuals across Ahoskie, Murfreesboro, Winton, and other areas within Hertford County.
Schedule:
- Flexible shifts available seven days a week
- Customize your schedule to fit personal and professional commitments
- Minimum required hours can be met weekly or bi-weekly
Responsibilities:
- Perform comprehensive home health risk assessments, each averaging 45 minutes per visit
- Assess and document prior and current health conditions, medical history, and medication compliance
- Educate patients regarding their medical conditions and prescribed medications
- Identify gaps in care, providing recommendations to help close these gaps
- Visits are focused on assessment onlyno treatment planning or medication prescribing required
Desired Experience & Qualifications:
- Active Nurse Practitioner license in North Carolina
- Board Certification through AANP or ANCC
- Current CPR (BLS) certification
- MSN degree from an accredited nursing program
- At least 6 months of recent experience as a Nurse Practitioner is preferred
This position offers competitive pay at $80-$90 per evaluation, plus mileage stipend and compensation for administrative time.
