Shape your career as a Nurse Practitioner specializing in home health by providing vital risk assessments to Medicare Advantage plan members across Washington County, NC. This per diem opportunity offers true scheduling flexibility, allowing you to design shifts that suit your availabilitybe it weekdays or weekendswith the freedom to fulfill minimum hourly commitments weekly or bi-weekly. You may conduct visits in various locations within Washington County, such as Plymouth, Roper, or Creswell, depending on shift assignments.
Schedule:
- Flexible shifts available Monday through Sunday
- Self-scheduling for optimal work-life balance
- Minimum weekly or bi-weekly hourly commitment
Key Responsibilities:
- Complete annual home health risk assessments in members homes
- Review and document medical histories, existing conditions, and medication use
- Educate patients about their health conditions and medication regimens
- Identify gaps in care and provide actionable recommendations to resolve them
- No responsibility for treatment planning or medication prescribing
Qualifications:
- Active and unrestricted Nurse Practitioner license in North Carolina
- Current CPR (BLS) certification
- Board Certification via AANP or ANCC
- Master of Science in Nursing (MSN) from an accredited program
- At least 6 months of experience as a Nurse Practitioner preferred
