Schedule:
- Flexible shifts available Monday through Sunday
- Create your own schedule
- Minimum weekly or bi-weekly hourly commitment
Key Responsibilities:
- Conduct annual risk assessments in patients homes, spending approximately 45 minutes per visit
- Evaluate prior and current medical histories, chronic or acute conditions, and medication use
- Educate patients on their medical conditions and proper medication usage
- Identify gaps in care and recommend appropriate follow-up actions
- Visits may include annual health reviews or post-discharge assessments
- No need to develop treatment plans or prescribe medication
Qualifications:
- Active Nurse Practitioner license in North Carolina
- Board certification through AANP or ANCC
- Current BLS/CPR certification
- MSN degree from an accredited nursing school
- At least six months of recent Nurse Practitioner experience (preferred)
- Commitment to delivering compassionate care in a home health setting
