Make a meaningful impact as a Nurse Practitioner by providing essential home health risk assessments for Medicare Advantage members across Hertford County, NC. Use your clinical expertise in a per diem role that values your flexibility and commitment. You will visit members in a variety of settingsincluding annual health assessments and post-discharge check-insensuring comprehensive evaluations that support better patient outcomes.
Schedule:
- Flexible shifts available Monday through Sunday
- Design your own work schedule to fit your lifestyle
- Minimum hourly commitment, either weekly or bi-weekly
Key Responsibilities:
- Conduct in-home annual risk assessments for Medicare Advantage plan members across locations such as Ahoskie, Murfreesboro, and Winton
- Review prior and current medical histories, conditions, and medications
- Identify gaps in care and recommend solutions, supporting overall patient health
- Educate members about their medical conditions and prescribed medications
- Focus exclusively on thorough assessmentno need to create treatment plans or prescribe medication
Qualifications:
- Nurse Practitioner license in North Carolina
- Active Board Certification through AANP or ANCC
- Basic Life Support (BLS) certification
- Master of Science in Nursing (MSN) from an accredited nursing school
- At least 6 months of recent Nurse Practitioner experience preferred
