Job Description

Opportunity available for an experienced Nurse Practitioner to provide home health risk assessments for Medicare Advantage plan members throughout Weld County, CO, including Windsor, Greeley, and Longmont. This per diem role offers the autonomy to create your own schedule, with flexible hours available seven days a week and a minimum commitment required on a weekly or bi-weekly basis.

Schedule:

  • Choose your own shifts between Monday and Sunday
  • Flexibility to build your own work schedule
  • Minimum weekly or bi-weekly hourly commitment

Responsibilities:

  • Perform comprehensive home health risk assessments, typically 45 minutes per visit, in patients homes
  • Review and evaluate medical histories, chronic conditions, and current medications
  • Educate patients about their health status, conditions, and medications
  • Identify gaps in care and recommend steps for improved health outcomes
  • No requirement to develop individual treatment plans or prescribe medications

Qualifications:

  • Current Nurse Practitioner license in Colorado
  • Board Certification through AANP or ANCC
  • MSN degree from an accredited nursing program
  • Active CPR certification (BLS)
  • At least 6 months of nurse practitioner experience preferred

This role offers competitive compensation of $80$90 per evaluation, including mileage reimbursement for travel between patient homes and compensation for administrative time spent charting.
