Key Responsibilities:

  • Perform annual home health risk assessments for Medicare Advantage members in their own homes
  • Assess medical history, current conditions, and medication usage comprehensively
  • Educate patients on their health conditions and prescribed medications
  • Identify care gaps and recommend actions to improve patient health outcomes
  • Focus exclusively on assessmentsno responsibility for creating treatment plans or prescribing medications

Required Qualifications:

  • Active Nurse Practitioner license in Connecticut
  • Master of Science in Nursing (MSN) from an accredited institution
  • Board Certification by AANP or ANCC
  • Current BLS certification
  • At least 6 months of experience as a Nurse Practitioner is preferred
