Key Responsibilities:
- Perform annual home health risk assessments for Medicare Advantage members in their own homes
- Assess medical history, current conditions, and medication usage comprehensively
- Educate patients on their health conditions and prescribed medications
- Identify care gaps and recommend actions to improve patient health outcomes
- Focus exclusively on assessmentsno responsibility for creating treatment plans or prescribing medications
Required Qualifications:
- Active Nurse Practitioner license in Connecticut
- Master of Science in Nursing (MSN) from an accredited institution
- Board Certification by AANP or ANCC
- Current BLS certification
- At least 6 months of experience as a Nurse Practitioner is preferred
