Advance your career by providing expert care in a home-based setting. This per diem opportunity is designed for a Nurse Practitioner with experience or interest in home health assessments. Youll conduct risk evaluations for Medicare Advantage plan members in areas such as Kissimmee, St. Cloud, and Poinciana within Osceola County, FL. Assess each member in their home, ensuring personalized attention and thorough evaluation.
Schedule:
- Flexible shifts available Monday through Sunday
- Create your own work schedule
- Choose a consistent weekly or bi-weekly commitment that fits your availability
Key Responsibilities:
- Complete annual home health risk assessments for Medicare Advantage members
- Review patients prior and current medical, medication, and condition history
- Educate members about their medical conditions and prescribed medications
- Identify and recommend solutions to resolve gaps in care
- Visits are focused on assessment onlyno treatment planning or prescription duties
Qualifications:
- Active Nurse Practitioner license in Florida
- Board Certification through AANP or ANCC
- MSN from an accredited nursing program
- 6 months or more of Nurse Practitioner experience preferred
- Current CPR/BLS certification
Compensation: Earn $80-$90 per evaluation, with an additional mileage stipend and compensated administrative time.
