A rewarding per diem opportunity is available for a skilled Nurse Practitioner to deliver in-home health risk assessments for Medicare Advantage members throughout Warren County, VA. This flexible role allows you to make a direct impact on patient health while maintaining the autonomy to create your own schedule. Evaluations may range from annual health assessments to post-discharge visits, with each assessment taking approximately 45 minutes. Candidates may be assigned to any location within Warren Countyincluding Bentonville and Front Royalduring individual shifts.

Schedule:

• Flexible shifts available Monday through Sunday
• Set your own schedule to fit your availability
• Minimum weekly or bi-weekly hourly commitment required

Key Responsibilities:

• Perform comprehensive annual home health risk assessments for Medicare Advantage plan members
• Review prior and current medical histories, ongoing conditions, and medication lists
• Educate patients on their health conditions and medication management
• Identify gaps in care and recommend strategies for ongoing wellness
• No requirement to create treatment plans or prescribe medication

Qualifications:

• Active Nurse Practitioner license in the state of Virginia
• Current CPR certification (BLS)
• Board Certified through AANP or ANCC
• A minimum of six months current experience as a Nurse Practitioner preferred
• MSN degree from an accredited school of nursing

This role offers competitive compensation of $80$90 per completed evaluation, along with a mileage stipend and additional paid administrative time.
