Advance your Nurse Practitioner career with flexible, per diem opportunities conducting home health risk assessments in the Coffee County, TN area. This role focuses on supporting Medicare Advantage members through comprehensive in-home visits that promote preventative care and patient education.

Schedule:

• Flexible shifts available between Monday and Sunday
• Design your own work schedule
• Minimum weekly or bi-weekly hourly requirement

Core Responsibilities:

• Perform annual and post-discharge home health risk assessments on Medicare Advantage plan members in Tullahoma, Manchester, Beechgrove, and surrounding areas
• Assess both prior and current medical history, medical conditions, and medication use
• Educate patients on health conditions and proper medication management during each 45-minute evaluation
• Detect care gaps and offer actionable recommendations to enhance patient outcomes
• Please note: This position does not involve developing treatment plans or prescribing medications

Required Qualifications:

• Active Nurse Practitioner license in Tennessee
• Board Certification through AANP or ANCC
• CPR (BLS) certification
• Graduation from an accredited MSN program
• At least 6 months of recent experience as a Nurse Practitioner is preferred
