Deliver essential healthcare services to Medicare Advantage plan members through in-home risk assessments. This per diem opportunity places you at the heart of patient care within Coffee County, where you can provide valuable evaluations and health education while enjoying a highly flexible work arrangement.
Schedule:
- Flexible shifts available any day, Monday through Sunday
- Design your own work schedule
- Minimum weekly or bi-weekly hourly commitment required
Core Responsibilities:
- Conduct annual home health risk assessments for members within designated areas of Coffee County
- Assess patients prior and current medical histories, ongoing conditions, and medications
- Deliver clear education about health conditions and prescribed medications
- Identify care gaps and offer actionable recommendations for improved wellness
- No responsibility for developing treatment plans or prescribing medications
Job Requirements:
- Active Nurse Practitioner license in Tennessee
- Board Certification via AANP or ANCC
- Current BLS (CPR) certification
- Completion of an MSN degree through an accredited nursing program
- Minimum of six months recent Nurse Practitioner experience preferred
Compensation: Earn between $80$90 for each evaluation, with additional mileage stipend and compensation for administrative duties, providing a robust opportunity to maximize income while maintaining work-life balance.
