Job Description

Advance your Nurse Practitioner career providing essential care to Medicare Advantage members through home health risk assessments. This per diem opportunity allows for a highly flexible schedule and autonomous work within Franklin County. Visits may include annual health assessments or post-discharge check-ins, ensuring thorough care across a range of patient needs.

Schedule:

  • Flexible shifts available Monday through Sunday
  • Self-scheduled based on your availability
  • Minimum weekly or bi-weekly hourly commitment

Core Responsibilities:

  • Complete comprehensive home health risk assessments for Medicare Advantage plan members
  • Review and evaluate patients medical histories, conditions, and medication usage
  • Deliver education regarding health conditions and prescribed medications
  • Identify gaps in care and offer actionable recommendations
  • Focus on assessmentno treatment planning or prescribing required

Qualifications:

  • Valid Nurse Practitioner license in Tennessee
  • Current CPR certification (BLS)
  • Board Certification via AANP or ANCC
  • Completion of MSN from an accredited nursing program
  • At least 6 months of recent NP experience preferred
