Advance your Nurse Practitioner career providing essential care to Medicare Advantage members through home health risk assessments. This per diem opportunity allows for a highly flexible schedule and autonomous work within Franklin County. Visits may include annual health assessments or post-discharge check-ins, ensuring thorough care across a range of patient needs.
Schedule:
- Flexible shifts available Monday through Sunday
- Self-scheduled based on your availability
- Minimum weekly or bi-weekly hourly commitment
Core Responsibilities:
- Complete comprehensive home health risk assessments for Medicare Advantage plan members
- Review and evaluate patients medical histories, conditions, and medication usage
- Deliver education regarding health conditions and prescribed medications
- Identify gaps in care and offer actionable recommendations
- Focus on assessmentno treatment planning or prescribing required
Qualifications:
- Valid Nurse Practitioner license in Tennessee
- Current CPR certification (BLS)
- Board Certification via AANP or ANCC
- Completion of MSN from an accredited nursing program
- At least 6 months of recent NP experience preferred
