Job Description

Step into a meaningful per diem opportunity providing in-home assessments for Medicare Advantage members across Coffee County, TN, including Tullahoma, Manchester, and Beechgrove. As a Nurse Practitioner specializing in home health, you will conduct thorough risk assessments, each taking about 45 minutes, focusing on annual health evaluations and post-discharge visits. This position allows you to make a direct impact on patient care while enjoying flexible scheduling tailored to your personal availability.

Schedule:

  • Flexible shifts available Monday through Sunday
  • Build your own schedule
  • Commitment to minimum weekly or bi-weekly hours

Key Responsibilities:

  • Complete comprehensive, in-home annual risk assessments for assigned Medicare Advantage plan members
  • Review patients prior and current medical history, chronic medical conditions, and medication regimens
  • Educate patients about their conditions and prescribed medications
  • Identify care gaps and suggest interventions and follow-up recommendations
  • No requirement to develop treatment plans or prescribe medications

Required Qualifications:

  • Active Nurse Practitioner (NP) license in Tennessee
  • Current CPR certification (BLS)
  • Board Certification through AANP or ANCC
  • MSN from an accredited nursing program
  • At least six months experience as a practicing Nurse Practitioner preferred
