Seeking a Nurse Practitioner with a passion for home health and flexibility, this per diem role offers opportunities to make a direct impact on the well-being of adult and senior patients in Jay County, IN. In this position, you will conduct in-home risk assessments within Portland, Dunkirk, Redkey, and surrounding communities. Each home health visitaveraging 45 minutescould include annual health checks or post-discharge assessments for Medicare Advantage plan members.

Schedule:

• Flexible shifts available Monday through Sunday

Key Responsibilities:

• Complete annual risk assessments for Medicare Advantage members in their homes
• Evaluate and review patients medical, surgical, and medication histories
• Educate patients on medical conditions and proper medication use
• Identify and address gaps in care, recommending next steps as needed
• No responsibility for ongoing treatment plans or prescribing medications

Qualifications:

• Active Nurse Practitioner license in Indiana
• Board certification via AANP or ANCC
• Master of Science in Nursing from an accredited institution
• CPR certification (BLS required)
• At least 6 months of recent NP experience is preferred

This role provides $80-$90 per evaluation, with additional compensation for mileage and administrative duties.
