Dynamic per diem opportunity for an experienced Nurse Practitioner to deliver home health risk assessments across Nash County, including Rocky Mount, Nashville, and Red Oak. With flexibility at the core, you can build your own schedule and select shifts throughout the week, including weekends, to fit your personal and professional commitments.

Schedule:

• Flexible shifts available Monday through Sunday
• Self-scheduling options
• Choose minimum weekly or bi-weekly commitments

Key Responsibilities:

• Complete comprehensive health risk assessments for Medicare Advantage plan members in their homes
• Evaluate prior and current medical history, chronic conditions, and medication regimens
• Educate patients about their medical conditions and prescribed medications
• Identify care gaps and collaborate to recommend resolutions
• No requirement to develop ongoing treatment plans or prescribe medications

Qualifications:

• Current Nurse Practitioner license in North Carolina
• Active Board Certification through AANP or ANCC
• CPR certification (BLS)
• MSN degree from an accredited nursing program
• Minimum of 6 months experience as a Nurse Practitioner preferred
• Strong communication skills and an independent, self-motivated work style
