Make a significant impact in home health by providing care to Medicare Advantage plan members across Nash County. This per diem opportunity is ideal for a Nurse Practitioner with a passion for community-based healthcare and flexibility in their work schedule.
Schedule:
- Flexible shifts available between Monday and Sunday
- Self-scheduling options for optimal work-life balance
- Minimum weekly or bi-weekly hourly commitment required
Key Responsibilities:
- Complete annual and post-discharge home health risk assessments for Medicare Advantage plan members residing in Rocky Mount, Nashville, and Red Oak
- Thoroughly evaluate each members medical history, current conditions, and medication usage
- Offer clear education on health conditions and medication management during each home visit
- Identify potential gaps in care and provide recommendations to address them
- No prescription writing or creation of ongoing treatment plans required, keeping each assessment focused and streamlined
Qualifications:
- Active Nurse Practitioner license in North Carolina
- Current CPR certification (BLS)
- Board certified through AANP or ANCC
- A minimum of six months Nurse Practitioner experience preferred
- MSN degree obtained from an accredited school of nursing
In this role, you will receive competitive compensation at $80-$90 per evaluation. Additionally, a mileage stipend and compensation for administrative time are provided. Enjoy the freedom to create your own schedule and work in diverse communities throughout Nash County.
