An exciting opportunity is available for Nurse Practitioners with a passion for home health care. In this per diem role, you will be conducting home health risk assessments with Medicare Advantage plan members throughout Nash County. Your expertise will directly support the health and well-being of community members, providing critical input without the pressure of treatment planning or prescription duties.
Schedule Details:
- Flexible shifts available Monday through Sunday
- The ability to build and control your own schedule
- Commitment can be set on a weekly or bi-weekly basis
Responsibilities:
- Complete annual health assessments and post-discharge visits in patients homes within Nash County, including Rocky Mount, Nashville, and Red Oak
- Conduct thorough evaluations of medical histories, medication regimens, and existing health conditions
- Offer clear education to patients about their health and medications
- Identify care gaps and suggest steps for improved outcomes
- Visits are structured (average 45 minutes); administrative time and local travel included
Qualifications:
- Current, unrestricted Nurse Practitioner license in North Carolina
- Master of Science in Nursing (MSN) from an accredited institution
- Board certification through AANP or ANCC
- Active CPR certification (BLS required)
- At least 6 months of recent experience as an NP preferred, but not required
This position offers a competitive rate of $80$90 per completed evaluation, with additional support including a mileage stipend and administrative time compensation.
