Job Description

Advance your career in home health as a dedicated Nurse Practitioner providing comprehensive risk assessments for Medicare Advantage plan members. This per diem opportunity allows you to maximize flexibility with self-selected shifts and makes an impact on individuals health in Wilson County communities, including Wilson, Elm City, and Lucama.

Schedule:

  • Flexible shifts offered Monday through Sunday
  • Design your own work schedule
  • Choose a minimum weekly or bi-weekly hourly commitment

Responsibilities:

  • Conduct thorough home health risk assessments, spending approximately 45 minutes per evaluation in client homes
  • Review and assess current and prior medical history, ongoing conditions, and medication use
  • Educate patients on their medical concerns, conditions, and medication regimens
  • Identify gaps in care and offer recommendations to support better outcomes
  • Visits may include annual health assessments or post-discharge follow-ups
  • No requirement to create treatment plans or prescribe medication

Qualifications:

  • Active Nurse Practitioner license in North Carolina
  • Master of Science in Nursing (MSN) from an accredited nursing school
  • Board Certification through AANP or ANCC
  • Current BLS (CPR) certification
  • At least 6 months of recent Nurse Practitioner experience preferred
