Advance your career in home health as a dedicated Nurse Practitioner providing comprehensive risk assessments for Medicare Advantage plan members. This per diem opportunity allows you to maximize flexibility with self-selected shifts and makes an impact on individuals health in Wilson County communities, including Wilson, Elm City, and Lucama.
Schedule:
- Flexible shifts offered Monday through Sunday
- Design your own work schedule
- Choose a minimum weekly or bi-weekly hourly commitment
Responsibilities:
- Conduct thorough home health risk assessments, spending approximately 45 minutes per evaluation in client homes
- Review and assess current and prior medical history, ongoing conditions, and medication use
- Educate patients on their medical concerns, conditions, and medication regimens
- Identify gaps in care and offer recommendations to support better outcomes
- Visits may include annual health assessments or post-discharge follow-ups
- No requirement to create treatment plans or prescribe medication
Qualifications:
- Active Nurse Practitioner license in North Carolina
- Master of Science in Nursing (MSN) from an accredited nursing school
- Board Certification through AANP or ANCC
- Current BLS (CPR) certification
- At least 6 months of recent Nurse Practitioner experience preferred
