Key Responsibilities:

  • Conduct comprehensive home health risk assessments for Medicare Advantage plan members in Monticello, Wilmar, Winchester, and surrounding areas within Drew County.
  • Evaluate patients' medical history, current health conditions, and medication profiles during visits averaging 45 minutes.
  • Educate patients about their health conditions and prescribed medications in their home environment.
  • Identify potential gaps in care and provide clear recommendations for follow-up and resolution.
  • Focus exclusively on assessmentno treatment plan creation or prescription responsibilities required.

Schedule:

  • Flexible shifts available Monday through Sunday
  • Design your own work schedule
  • Commit to a minimum number of hours weekly or bi-weekly

Required Qualifications:

  • Active Nurse Practitioner License in Arkansas
  • Current BLS/CPR certification
  • Board Certification (AANP or ANCC)
  • MSN from an accredited nursing program
  • At least six months of current NP experience preferred
