Job Description
Healthcare professionals with expertise in Home Health and Nurse Practitioner roles are needed for per diem opportunities conducting home-based risk assessments within Drew County. This position offers the chance to provide valuable care for Medicare Advantage plan members living in Monticello, Wilmar, Winchester, and neighboring communities. Assessments typically take about 45 minutes per visit, which may consist of annual health evaluations or post-discharge assessments.
Schedule:
- Flexible shifts available Monday through Sunday
- Design your own work schedule
- Weekly or bi-weekly hourly commitment required
Key Responsibilities:
- Complete annual home health risk assessments for Medicare Advantage plan members
- Review and evaluate prior and current medical histories, chronic conditions, and all prescribed medications
- Offer patient education related to their medical conditions and medication regimens
- Identify healthcare gaps and recommend actions to address them
- No treatment planning or prescription responsibilities
Qualifications:
- Valid Nurse Practitioner license in Arkansas
- Active CPR (BLS) certification
- National Board Certification through AANP or ANCC
- Minimum of 6 months recent experience working as a Nurse Practitioner preferred
- Master of Science in Nursing (MSN) from an accredited program
For each completed evaluation, you will receive compensation of $80$90, in addition to mileage reimbursement and a stipend for administrative time.
