Deliver advanced patient care and make a significant impact in community health by providing expert home health risk assessments. The position centers on conducting thorough evaluations for Medicare Advantage plan members in Howell County, which may include annual health reviews and post-discharge assessments. Flexible per diem scheduling allows for professional autonomy, with visits taking place in West Plains, Mountain View, Willow Springs, and other nearby locations.
Schedule:
- Flexible shifts available Monday through Sunday
- Create your own schedule to fit your needs
- Minimum weekly or bi-weekly hourly commitment
Key Responsibilities:
- Complete comprehensive home health risk assessments for Medicare Advantage plan members
- Review and document medical history, ongoing conditions, and current medication usage
- Provide targeted education regarding specific medical conditions and proper medication management
- Identify and communicate gaps in care, offering actionable recommendations for improvement
- Note: This role does not involve creating treatment plans or prescribing medications
Qualifications:
- Nurse Practitioner license valid in Missouri
- Active CPR certification (BLS required)
- Board Certification through AANP or ANCC
- MSN degree from an accredited nursing institution
- Minimum of 6 months recent NP experience preferred
