Make a meaningful impact by leveraging your clinical expertise to perform home health risk assessments for Medicare Advantage plan members across Harnett County, NC. This flexible, per diem opportunity is ideal for Nurse Practitioners who excel at delivering compassionate, comprehensive in-home care.
Schedule:
- Flexible shifts available Monday through Sunday
- Design your own schedulechoose shifts that fit your lifestyle
- Minimum weekly or bi-weekly hourly commitment
Responsibilities:
- Conduct thorough annual or post-discharge home health risk assessments, each lasting approximately 45 minutes
- Assess patients medical histories, current health conditions, and medication profiles
- Deliver patient education regarding conditions and medications, promoting overall wellness
- Proactively identify care gaps and suggest strategies for improved patient outcomes
- These visits do not involve developing treatment plans or prescribing medications
Qualifications:
- Active Nurse Practitioner license in NC
- Board Certified by AANP or ANCC
- Current CPR (BLS) certification
- MSN from an accredited nursing program
- At least 6 months of recent Nurse Practitioner experience preferred
