Job Description
Deliver high-quality patient care and make a meaningful difference as a Nurse Practitioner providing home health risk assessments for Medicare Advantage members. This per diem opportunity offers true flexibility, allowing you to create your own schedule and achieve a satisfying work-life balance while supporting individuals in their homes throughout Beaufort County.
Schedule:
- Flexible shifts available Monday through Sunday
- Set your preferred schedule
- Minimum weekly or bi-weekly hour commitments to suit your availability
Key Responsibilities:
- Complete risk assessments in patients homes, including annual and post-discharge visits
- Evaluate patients prior and current medical history, medical conditions, and medication use
- Educate patients on managing medical conditions and understanding prescribed medications
- Identify care gaps, provide recommendations for improvement, and ensure thorough documentation
- No responsibility for developing treatment plans or prescribing medications
Qualifications:
- Active Nurse Practitioner license issued by the state of North Carolina
- Current CPR certification (BLS)
- Board certification through AANP or ANCC required
- MSN degree from an accredited institution
- At least six months of NP professional experience preferred
