Job Description

Deliver high-quality patient care and make a meaningful difference as a Nurse Practitioner providing home health risk assessments for Medicare Advantage members. This per diem opportunity offers true flexibility, allowing you to create your own schedule and achieve a satisfying work-life balance while supporting individuals in their homes throughout Beaufort County.

Schedule:

  • Flexible shifts available Monday through Sunday
  • Set your preferred schedule
  • Minimum weekly or bi-weekly hour commitments to suit your availability

Key Responsibilities:

  • Complete risk assessments in patients homes, including annual and post-discharge visits
  • Evaluate patients prior and current medical history, medical conditions, and medication use
  • Educate patients on managing medical conditions and understanding prescribed medications
  • Identify care gaps, provide recommendations for improvement, and ensure thorough documentation
  • No responsibility for developing treatment plans or prescribing medications

Qualifications:

  • Active Nurse Practitioner license issued by the state of North Carolina
  • Current CPR certification (BLS)
  • Board certification through AANP or ANCC required
  • MSN degree from an accredited institution
  • At least six months of NP professional experience preferred
