Make a meaningful impact in patient care by performing in-home health risk assessments for Medicare Advantage plan members. This per diem opportunity offers flexibility with shifts available between Monday and Sunday, allowing you to structure your work schedule to fit your lifeincluding options for creating your own schedule and meeting minimum hourly commitments weekly or bi-weekly.

Responsibilities:

• Conduct home health risk assessments, typically lasting 45 minutes, for plan members in their residences.
• Review and evaluate past and current medical history, conditions, and medication use.
• Educate patients about their health conditions and medication regimens during each visit.
• Identify care gaps and provide actionable recommendations for follow-up without responsibility for creating treatment plans or prescribing medications.
• Document findings and maintain detailed, compliant records for each encounter.

Required Qualifications:

• Active Nurse Practitioner license in Virginia
• Current CPR certification (BLS)
• Board Certification from AANP or ANCC
• MSN degree from an accredited program
• At least six months of recent NP experience is preferred

This role offers $80$90 per completed evaluation, plus a mileage stipend and compensation for administrative time. Youll have the autonomy to organize your visits while supporting better health outcomes for individuals in the community.
