Embark on a meaningful per diem role where your expertise as a Nurse Practitioner makes a tangible difference in patients' lives. This home health opportunity centers on conducting home health risk assessments for Medicare Advantage plan members in Williamsburg and the surrounding James City County area, including notable locations such as Williamsburg and Jamestown. Youll play a pivotal part in helping patients access preventive healthcare, while enjoying the flexibility to design your own schedule.

Schedule:

• Flexible shifts available Monday through Sunday
• Set your own work hours
• Minimum hourly commitment required weekly or bi-weekly

Key Responsibilities:

• Perform annual risk assessments in patients homes, each typically lasting about 45 minutes
• Review prior and current health histories, chronic conditions, and medications
• Deliver education to patients regarding their conditions and prescribed medications
• Identify care gaps and recommend actions for improved patient outcomes
• No requirements for creating treatment plans or writing prescriptions

Preferred Qualifications:

• Active Nurse Practitioner License in Virginia
• Board Certification via AANP or ANCC
• Current BLS CPR certification
• MSN from an accredited nursing program
• At least six months of recent NP experience (preferred)
