Job Description
Make a meaningful impact by providing in-home health assessments for Medicare Advantage plan members residing in Wilkes County. This per diem opportunity allows you to deliver personalized care while enjoying the flexibility to shape your own schedule.
Schedule:
- Flexible shifts available seven days a week
- Create your own schedule
- Choose your preferred minimum weekly or bi-weekly hourly commitment
Key Responsibilities:
- Perform comprehensive home health risk assessments, each lasting approximately 45 minutes
- Evaluate patients medical and medication histories, as well as prior and current conditions
- Educate patients on their health conditions and prescribed medications
- Identify gaps in care and provide actionable recommendations
- No responsibilities for medication prescribing or the creation of treatment plans
Qualifications:
- Active Nurse Practitioner License in North Carolina
- Board Certified through AANP or ANCC
- Current BLS/CPR certification
- MSN degree from an accredited nursing school
- At least six months of current Nurse Practitioner experience preferred
This role offers a competitive rate of $80-$90 per completed evaluation, in addition to a mileage stipend and compensation for administrative time.
