Deliver vital healthcare services directly to patients homes as an experienced Nurse Practitioner providing home health risk assessments. Schedule:

• Choose your own shifts between Monday and Sunday
• Flexible start and end times
• Minimum weekly or bi-weekly hourly commitment

Key Responsibilities:

• Conduct comprehensive annual home health risk assessments for Medicare Advantage plan members in their residences throughout Wichita Falls and surrounding areas, including Burkburnett, Electra, and Kamay.
• Review and evaluate patients prior and current medical history, chronic and acute medical conditions, and medication usage.
• Educate patients on their health conditions and prescribed medications.
• Identify gaps in care, making recommendations to ensure continuity and quality of care.
• Visits focus on assessment and education; no treatment plans or prescriptions are required.

Qualifications:

• Active Nurse Practitioner license in Texas
• Board Certification through AANP or ANCC
• Graduate of an accredited MSN program
• Current CPR/BLS certification
• At least 6 months of Nurse Practitioner experience preferred

This role offers compensation of $80-$90 per evaluation, plus a mileage stipend and consideration for administrative time.
