Job Description

In this position, you'll have the unique opportunity to visit Medicare Advantage plan members in their homes, performing comprehensive risk assessments that contribute to improved patient outcomes and quality of care. Schedule:

  • Flexible shifts available Monday through Sunday
  • Design your own schedule
  • Minimum weekly or bi-weekly hourly commitment

Key Responsibilities:

  • Perform annual home health risk assessments for Medicare Advantage members
  • Evaluate patient medical history, current conditions, and medication usage
  • Educate patients regarding their health conditions and prescribed medications
  • Identify care gaps and recommend solutions; direct treatment planning is not required
  • Visits take approximately 45 minutes and may include annual assessments or post-discharge visits
  • Travel to various locations within Columbus County, potentially including Tabor City, Chadbourn, and Lake Waccamaw

Qualifications:

  • Active Nurse Practitioner license in North Carolina
  • Board certification (AANP or ANCC)
  • Current CPR/BLS certification
  • MSN from an accredited nursing school
  • At least 6 months' experience as an NP preferred
