Exciting per diem opportunity available for a skilled Nurse Practitioner to deliver home health risk assessments throughout Oneida County, NY. This position focuses on in-home evaluations for Medicare Advantage plan members located in communities such as Utica, Rome, Whitesboro, and New York Mills. Flexible scheduling enables you to create shifts that fit your lifestyle, with required coverage available any day of the week.

Schedule:

• Flexible shifts available Monday through Sunday
• Choose your own schedule
• Minimum weekly or bi-weekly hourly commitment

Key Responsibilities:

• Perform comprehensive annual and post-discharge home health risk assessments (average 45 minutes per assessment)
• Review prior and current medical history, existing medical conditions, and medications
• Educate members about their health conditions and prescribed medications
• Identify and report gaps in care, providing recommendations for follow-up
• No responsibility for creating treatment plans or prescribing medications

Qualifications:

• Active Nurse Practitioner license in New York
• Board certification required (AANP or ANCC)
• Current CPR (BLS) certification
• Minimum of six months recent NP experience preferred
• MSN from an accredited nursing program
