Make a meaningful impact in the community by performing home health risk assessments for Medicare Advantage plan members throughout Fulton County. In this per diem nurse practitioner role, youll travel to various locationssuch as Wauseon, Archbold, and Swantonto deliver thorough, patient-centered evaluations. Each visit typically lasts about 45 minutes and may include annual health assessments or post-discharge check-ins.

• Flexible shifts available Monday through Sunday

• Choose minimum weekly or bi-weekly hourly commitments

What Youll Do:

• Carry out comprehensive home health risk assessments for Medicare Advantage members
• Review prior and current medical history, existing conditions, and medication use
• Educate patients regarding diagnoses and prescribed medications
• Spot any gaps in care and offer evidence-based recommendations
• Documentation onlyno need to prescribe, order treatments, or develop care plans

Requirements:

• Active nurse practitioner license in Ohio
• Board certification from AANP or ANCC
• Current CPR certification (BLS)
• MSN degree from an accredited nursing program
• At least 6 months experience as a nurse practitioner preferred
• Ability to travel within Fulton County to perform in-home assessments

Compensation: Earn $80 to $90 per completed evaluation, with additional mileage reimbursement and compensation for administrative time.
