Take on an impactful role delivering essential healthcare services to Medicare Advantage plan members through home health risk assessments. This per diem Nurse Practitioner opportunity offers you the autonomy to create your own schedule, with flexible hours available between Monday and Sunday and the ability to commit to minimum weekly or bi-weekly shifts. Serve diverse communities within Muskegon County as you engage with patients in their homes, supporting their health journey.
Key Responsibilities:
- Conduct comprehensive, 45-minute home health risk assessments to evaluate medical history and conditions for Medicare Advantage members.
- Review and document prior and current health concerns, including medication histories.
- Educate patients on their medical conditions and prescribed medications.
- Identify unaddressed care needs and recommend appropriate actions to close care gaps.
- Participate in annual assessments or post-discharge visits as needed.
- No requirement to create treatment plans or prescribe medications.
Qualifications:
- Active Nurse Practitioner license in Michigan.
- Board Certification through AANP or ANCC.
- Current BLS (CPR) certification.
- MSN degree from an accredited institution.
- At least 6 months of Nurse Practitioner experience is preferred.
