Job Description

Seeking a dedicated healthcare professional to perform comprehensive home health risk assessments for Medicare Advantage plan members across Muskegon County, MI. This per diem opportunity is ideal for Nurse Practitioners who appreciate autonomy and flexibility, as you will manage your own schedule and work with patients in their homes, focusing on thorough evaluations without ongoing clinical management or prescription responsibilities.

Schedule:

  • Flexible shifts available Monday through Sunday

  • Minimal weekly or bi-weekly hour commitment

Key Responsibilities:

  • Perform annual health risk assessments or post-discharge visits in patient homes
  • Assess prior and current medical history, chronic conditions, and medication usage
  • Educate patients about their health conditions and medications to encourage self-management
  • Identify care gaps and make recommendations to ensure continuity and quality of care
  • No treatment plan development or prescription writing required

Qualifications:

  • Active, unrestricted Nurse Practitioner license in Michigan
  • Board Certification as an NP through AANP or ANCC
  • Masters degree (MSN) from an accredited nursing program
  • Current CPR (BLS) certification
  • At least 6 months of recent NP experience preferred

Compensation for this position is $80-$90 per evaluation, with additional mileage stipend and pay for administrative time.
