Step into a meaningful per diem opportunity dedicated to making a difference in patients lives through home health risk assessments across Rio Arriba County. As a Nurse Practitioner specializing in Home Health, you will conduct comprehensive evaluations for Medicare Advantage plan members residing in communities such as Espaola, Chama, and Abiqui. Each assessmentwhether its an annual check-in or a post-discharge visittypically lasts about 45 minutes and focuses on ensuring clients receive the highest standard of care.

Schedule:

• Flexible shifts available Monday through Sunday
• Create your own schedule to fit your lifestyle
• Minimum hourly commitment can be weekly or bi-weekly

Responsibilities:

• Conduct detailed annual risk assessments in members homes
• Review and document current and prior medical history, conditions, and medication usage
• Provide tailored education about health conditions and prescribed medications
• Identify and relay any potential gaps in care and recommend solutions
• No medication prescribing or treatment plan creation required

Qualifications:

• Active Nurse Practitioner license in NM
• Current BLS (CPR) certification
• Certification through AANP or ANCC
• MSN degree from an accredited nursing program
• At least 6 months of recent NP experience preferred

This per assessment opportunity offers $80-$90 for each completed home visit, along with a mileage stipend and administrative time compensation, all structured to recognize the quality and expertise of your work.
