Deliver essential care and make a difference as a Nurse Practitioner specializing in home health risk assessments within Jefferson County, AR. This per diem role offers autonomy and the chance to tailor your schedule, making it an excellent fit for professionals seeking flexibility and meaningful patient interaction.
Schedule:
- Flexible shifts available Monday through Sunday
- Self-scheduling to fit your lifestyle
- Minimum commitment required on a weekly or bi-weekly basis
Responsibilities:
- Conduct comprehensive home health risk assessments for Medicare Advantage plan members across areas such as Pine Bluff, White Hall, Redfield, and beyond
- Gather and review data relating to prior and current medical history, ongoing conditions, and current medications
- Deliver patient education regarding diagnoses, preventive care, and medication regimens
- Pinpoint gaps in care and offer clear, actionable recommendations for follow-up (note: no prescribing or development of treatment plans required)
- Each assessment typically lasts around 45 minutes, allowing for thorough patient engagement
- Visits may be annual assessments or post-discharge follow-ups
- Receive a per-evaluation payment plus mileage and administrative time compensation
Qualifications:
- Active Nurse Practitioner license in Arkansas
- Board Certification through AANP or ANCC
- Completion of MSN degree from an accredited nursing program
- Current CPR (BLS) certification
- Minimum of six months of Nurse Practitioner experience preferred
