Deliver high-quality patient care as a Nurse Practitioner specializing in home health risk assessments across Jackson County, OR. Work per diem with extensive flexibilitychoose your own schedule and shifts any day between Monday and Sunday, with the ability to commit as your availability allows. Each patient visit typically takes around 45 minutes, and assignments span Medford and neighboring communities such as Ashland, Eagle Point, and Jacksonville.

Primary Responsibilities:

• Complete thorough annual health and post-discharge risk assessments for Medicare Advantage plan members in their homes
• Assess and document comprehensive medical histories, existing medical conditions, and medication use
• Educate patients on disease management and proper medication usage
• Pinpoint gaps in care and offer actionable recommendations to improve patient outcomes
• Visits focus on evaluationno development of treatment plans or prescribing required

Qualifications:

• Active Nurse Practitioner license in Oregon
• Board certification through AANP or ANCC
• MSN from an accredited nursing program
• Current CPR (BLS) certification
• Minimum of 6 months NP experience preferred
