Job Description

Schedule:

  • Flexible shifts between Monday and Sunday
  • Create your own schedule
  • Minimum weekly or bi-weekly hourly commitment

Primary Responsibilities:

  • Perform annual home health risk assessments, including both routine health evaluations and post-discharge visits
  • Review and evaluate past and current medical history, ongoing conditions, and medications
  • Educate patients on their health conditions and medication regimens
  • Identify gaps in care and provide actionable recommendations to enhance patient outcomes
  • No responsibility for developing treatment plans or prescribing medications

Desired Qualifications:

  • Active Nurse Practitioner License in Idaho
  • Current CPR (BLS) certification
  • Board Certification (AANP or ANCC)
  • Minimum of 6 months current NP experience preferred
  • MSN degree from an accredited nursing program

This position offers a competitive per-evaluation rate ranging from $80-$90, along with a mileage stipend and compensation for administrative time. The nature of the visits varies, ensuring an engaging and impactful workday as you visit Medicare Advantage members in locations such as Grangeville, Cottonwood, Kamiah, and other areas within Idaho County.
