Job Description

Advance your career in home health as a Nurse Practitioner providing valuable risk assessments to Medicare Advantage members in Edgecombe County, NC. This per diem opportunity offers the flexibility to tailor your schedule, with shifts available between Monday and Sunday and the ability to create your own hours. Candidates may be assigned to various locations within Edgecombe County during a shift, including communities such as Tarboro, Lower Town Creek, and Princeville.

Schedule:

  • Flexible shifts available Monday through Sunday
  • Self-scheduling to fit your availability
  • Minimum weekly or bi-weekly hourly commitment required

Primary Responsibilities:

  • Perform comprehensive annual home health risk assessments for Medicare Advantage plan members
  • Review and assess medical history, existing medical conditions, and current medication regimens
  • Provide clear education to patients on their medical conditions and medications
  • Identify care gaps and make recommendations to address them
  • Visit locations across Edgecombe County, as assigned per shift
  • No prescribing or treatment planning responsibilities

Required Qualifications:

  • Active Nurse Practitioner License in North Carolina
  • Master of Science in Nursing (MSN) from an accredited institution
  • Board Certification by AANP or ANCC
  • Valid CPR certification (BLS)
  • At least 6 months of NP experience strongly preferred

Compensation includes $80-$90 for each evaluation completed, along with a mileage stipend and administrative pay for documentation and clerical tasks.
