As a Nurse Practitioner providing essential home health risk assessments for Medicare Advantage plan members across Scioto County, Ohio, you play a pivotal role in improving patient outcomes. Assessment visits may occur anywhere within Wheelersburg, New Boston, Minford, Portsmouth, or other neighboring communities.
Schedule:
- Flexible shifts available Monday through Sunday
- Design your own work schedule
- Commit to a minimum number of weekly or bi-weekly hours, based on your availability
Key Responsibilities:
- Perform annual home health risk assessments for Medicare Advantage health plan members
- Conduct thorough reviews of medical history, current conditions, and medication regimens
- Educate patients about their medical conditions and prescribed medications
- Identify and communicate any care gaps, offering recommendations to support improved patient outcomes
- No requirement to develop treatment plans or prescribe medications
Required Qualifications:
- Active Nurse Practitioner license in Ohio
- Board Certification (AANP or ANCC)
- Current CPR/BLS certification
- MSN degree from an accredited nursing school
- At least 6 months of experience practicing as a Nurse Practitioner (preferred)
