Job Description

Deliver high-quality care directly in patients homes as a Nurse Practitioner specializing in home health risk assessments throughout Seminole County, OK. This unique per diem opportunity allows you to set your own schedule with flexible shifts available between Monday and Sunday, providing exceptional work-life balance. Youll have a minimum weekly or bi-weekly hourly commitment, but the power to customize shifts that work for you.

Key Responsibilities:

  • Conduct annual home health risk assessments for Medicare Advantage plan members
  • Evaluate prior and current medical histories, medical conditions, and medication use
  • Educate patients on their medical conditions and medications to promote better health understanding
  • Identify gaps in patient care and offer actionable recommendations
  • Perform assessments in various communities across Seminole CountyWewoka, Bowlegs, Konawa, and surrounding areas
  • Visits may include annual assessments or post-discharge follow-up; each assessment averages around 45 minutes
  • No requirement to develop treatment plans or prescribe medications

Qualifications:

  • Active Nurse Practitioner License in OK
  • Current CPR certification (BLS)
  • Board Certification through AANP or ANCC
  • 6 months of current NP experience preferred
  • MSN degree from an accredited school of nursing
