Deliver meaningful care in a flexible, per diem position as a Nurse Practitioner specializing in home health risk assessments. In this dynamic role, youll meet Medicare Advantage plan members throughout Calcasieu County, including Lake Charles, Sulphur, and DeQuincy, supporting patients directly in their homes. Each assessment typically lasts about 45 minutes and may focus on annual health reviews or post-discharge follow-ups. Your insights will help ensure members receive comprehensive evaluations and recommendations that promote their well-being.

Schedule:

• Flexible shifts available Monday through Sunday
• Design your own schedule to balance work and life needs
• Commit to a minimum number of weekly or bi-weekly hours

Key Responsibilities:

• Perform annual and post-discharge risk assessments for Medicare Advantage members within Calcasieu County
• Assess patients' medical and medication histories
• Educate individuals on their medical conditions and care regimens
• Identify care gaps and make recommendations for addressing them
• This role involves evaluation onlyno treatment planning or prescribing

Qualifications:

• Active Nurse Practitioner License in Louisiana
• Current BLS/CPR certification
• Board Certification through AANP or ANCC required
• A minimum of 6 months of NP experience preferred
• MSN from an accredited nursing program
