This flexible role offers autonomy in schedulingchoose shifts between Monday and Sunday that work best for you, with minimum weekly or bi-weekly commitments. You may conduct assessments throughout Springfield, Chicopee, Westfield, and surrounding areas.

Key responsibilities include:

• Performing comprehensive annual health risk assessments in patients homes
• Reviewing previous and current medical history, existing medical conditions, and all medications
• Delivering clear education to patients regarding their health conditions and medication usage
• Detecting and reporting gaps in care, while recommending actionable solutions
• Completing all documentation and evaluations; treatment planning or prescribing medications is not required

Qualifications for this position:

• Active Nurse Practitioner license in Massachusetts
• Board Certification (AANP or ANCC)
• Current BLS (CPR) certification
• At least six months of recent experience as a Nurse Practitioner preferred
• MSN degree from an accredited nursing institution
