Step into a dynamic per diem Nurse Practitioner opportunity focused on home health risk assessments in Hampden County, Massachusetts. In this independent role, you will visit Medicare Advantage plan members in their residencesserving communities such as Springfield, Chicopee, and Westfield. Assessments typically take about 45 minutes and may include annual health checks or post-discharge follow-ups, with no requirement to formulate treatment plans or prescribe medications.
Schedule:
- Flexible shifts available any day, Monday through Sunday
- Freedom to design your own schedule
- Minimum weekly or bi-weekly hourly commitment
Key Responsibilities:
- Perform home health risk assessments for Medicare Advantage members
- Evaluate medical histories, current conditions, and medication usage
- Educate members on their health conditions and prescribed medications
- Identify care gaps and offer recommendations to resolve them
- Complete documentation and participate in quality improvement processes
- Travel to patient residences across Hampden County, with shifts potentially scheduled in different towns
Required Qualifications:
- Active Nurse Practitioner license in Massachusetts
- Board certification through AANP or ANCC
- Current CPR certification (BLS)
- MSN degree from an accredited nursing program
- Minimum of 6 months recent nurse practitioner experience preferred
- Strong organizational and communication skills
- Reliable transportation for travel within Hampden County
