This per diem role offers flexibility, allowing you to design your own schedule with shifts available every day of the week. Work close to home as you provide essential healthcare evaluations in communities like Carmel, Fishers, and Noblesville.
Schedule:
- Flexible shifts available Monday through Sunday
- Design your own schedule to fit your needs
- Commitment required on a weekly or bi-weekly basis
Key Responsibilities:
- Perform annual and post-discharge home health risk assessments, averaging 45 minutes per visit
- Review patients medical history, current conditions, and medication use
- Deliver personalized education to members regarding their health status and medication regimen
- Identify gaps in care and provide actionable recommendations for improved health outcomes
- Focus strictly on assessmentsno need to develop treatment plans or prescribe medications
Qualifications:
- Active Nurse Practitioner license in Indiana
- Board Certification through AANP or ANCC required
- Master of Science in Nursing (MSN) from an accredited institution
- Current CPR certification (BLS)
- At least six months of recent nurse practitioner experience preferred
