Make a meaningful impact in home health by leveraging your expertise as a Nurse Practitioner on a per diem basis. In this rewarding and flexible role, you will conduct home health risk assessments for Medicare Advantage plan members in various neighborhoods of Washington County, RIincluding South Kingstown, Narragansett, Richmond, and Westerly. This work provides essential insights into members health while giving you the autonomy to manage your own schedule.

Schedule:

• Flexible shifts available every day of the week (Monday through Sunday)
• Create your own schedule based on your availability
• Commit to a minimum number of hours weekly or bi-weekly

Key Responsibilities:

• Perform annual and post-discharge home health risk assessments during 45-minute in-home visits
• Review and evaluate prior and current medical histories, existing medical conditions, and medication usage
• Provide patient education regarding health conditions and prescribed medications
• Identify gaps in care and recommend strategies to address them
• No requirement to create treatment plans or prescribe medications

Qualifications:

• Active Nurse Practitioner (NP) license in the state of Rhode Island
• Board Certification through AANP or ANCC
• Master of Science in Nursing (MSN) from an accredited program
• Current CPR certification (BLS)
• Minimum of 6 months of recent NP experience preferred

This position offers competitive compensation of $80 to $90 per evaluation, in addition to a mileage stipend and compensation for administrative time.
