Leverage your clinical expertise and autonomy in a per diem Nurse Practitioner opportunity focused on home health risk assessments throughout Clay County, IN. This position places you at the forefront of preventive healthcare for Medicare Advantage members living in Brazil, Clay City, Knightsville, and nearby communities.

Schedule:

• Flexible shifts available all days, Monday through Sunday
• Create your own work schedule for maximum convenience
• Commitment to a minimum number of hours weekly or bi-weekly

Key Responsibilities:

• Conduct comprehensive annual risk assessments for Medicare Advantage plan members in their homes
• Review and evaluate prior and current medical histories, health conditions, and detailed medication listings
• Provide patient education regarding chronic and acute medical conditions as well as guidance on medication usage
• Assess for potential gaps in patient care and give recommendations to improve overall health outcomes
• Focus strictly on assessment and educationno treatment planning or prescribing involved

Qualifications:

• Active Nurse Practitioner license in Indiana
• Current CPR/BLS certification
• Board certification through AANP or ANCC
• MSN degree from an accredited nursing school
• At least 6 months of current Nurse Practitioner experience (preferred)

This per diem engagement offers competitive compensation of $80-$90 per evaluation, complemented by a mileage stipend and administrative time coverage.
