Job Description
Deliver high-quality, patient-centered care through home health risk assessments as a Nurse Practitioner on a flexible, per diem basis. Serve Medicare Advantage plan members in Vigo County, conducting visits that support preventative healthcare and overall wellness.
Schedule:
- Flexible shifts, available any day Monday through Sunday
- Design your own work schedule
- Minimum weekly or bi-weekly hourly commitment, tailored to your availability
Key Responsibilities:
- Perform comprehensive annual and post-discharge home health risk assessments for Medicare Advantage members
- Assess prior and current medical history, health conditions, and medication usage
- Educate patients on their medical conditions and medications to encourage optimal self-care
- Identify gaps in care and make clinical recommendations for improvement
- Documentation and communicationno direct treatment planning or prescribing required
Qualifications:
- Valid Nurse Practitioner license (IN)
- Board Certification via AANP or ANCC
- Current CPR (BLS) certification
- Master of Science in Nursing (MSN) from an accredited program
- At least six months of Nurse Practitioner experience preferred
